Midface Lift (Cheek Lift) in Istanbul, Turkey
Short answer: A midface lift (cheek lift) is a facelift technique that specifically targets sagging in the cheek area. It aims to lift the descended fat pads over the cheekbones back to their original position and is often performed together with lower eyelid surgery through the same incision. Whether it suits you, and which technique and incision are used, is decided at a consultation in Nişantaşı, Istanbul.

What causes midface sagging?
Here, the midface means the cheeks, the cheekbones and the eyelid–cheek junction. Ageing affects its deeper layers, not only the skin:
- Volume loss: cheek fat pads thin and shift downwards; the cheekbones lose some volume.
- Loosening support: as the SMAS and the retaining ligaments anchoring it to bone loosen, the cheeks descend and the nasolabial folds deepen. The ligaments around the cheekbone help determine the position of the midface.
- Under-eye changes: lower eyelid fat bulges forwards to form bags, and a groove appears where the eyelid meets the cheek.
What does a midface lift aim to correct?
- Downward sagging in the cheek area
- The position of descended fat pads over the cheekbones
- With lower eyelid surgery: under-eye bags, the groove beneath them and the eye–cheek transition
It is a regional procedure. Jowls and neck laxity fall within facelift surgery; brows and volume loss alone may need a brow lift or fat grafting instead.
Midface lift techniques
Every face ages differently, so Assoc. Prof. Dr. Yalçın Bayram creates a personalised surgical plan.
Together with lower eyelid surgery
A midface lift is often performed together with lower eyelid surgery, through the same incision used for lower eyelid surgery. The type and position of the incision are decided at consultation after assessing lower eyelid tension, excess skin and under-eye bags. For more on lower eyelid incisions, see eyelid surgery.
With a deep plane facelift
In a deep plane facelift, the retaining ligaments around the cheekbone and chewing muscle are released and the skin and SMAS are repositioned as a single unit, allowing more movement in the midface and nasolabial area. In SMAS techniques such as plication and SMASectomy, tissue is pulled against the ligaments, so the effect on these areas may be more limited. See Deep Plane Facelift vs SMAS Lift.
Volume support: fat grafting
Fat grafting restores volume rather than lifting: your own purified fat is injected into areas such as the cheeks, under-eye area and nasolabial folds. Because a face that has lost volume can look empty if only tightened, it can be planned alongside a lift.
How the technique and incision are chosen
Whether a midface lift with lower eyelid surgery, a deep plane facelift or volume support is the main option for your midface is decided at consultation, based on a facial analysis, together with the type and position of the incision and any added procedures.
Midface lift, fat grafting or deep plane facelift?
A general framework; the decision is made at consultation.
| Criterion | Midface lift | Facial fat grafting | Deep plane facelift |
|---|---|---|---|
| Main concern | Sagging over the cheeks, eyelid–cheek transition | Volume loss: flattened cheeks, under-eye hollows, temples | Midface descent with jowls and neck laxity |
| What it does | Lifts descended fat pads | Fills hollows with your own fat | Releases ligaments, repositions skin and SMAS together |
| Often planned with | Lower eyelid surgery | A facelift, if sagging is present | Neck lift, eyelid surgery, fat grafting |
| Return to social life | Decided at consultation | Most patients by the end of weeks 1–2, possibly with concealer | Most patients within 2–3 weeks |

When sagging and volume loss occur together, fat grafting can be planned with a lift or separately. See fat grafting vs fillers.
Who may be a good candidate?
Suitability is decided at consultation, where the source of sagging, skin quality, volume loss, brow position and lower eyelid tension are assessed together. A midface lift is generally considered for people with:
- Noticeable sagging of the cheeks and over the cheekbones, sometimes with under-eye bags and a groove
- Good general health
- No smoking, or the ability to stop around surgery and during recovery
- Realistic expectations and a wish for a natural outcome
When another option may fit better
- No significant sagging, mainly volume loss or expression lines: botox, fillers or fat grafting may be discussed.
- Marked jowls and neck laxity: a facelift and neck lift come to the fore.
- Low brows: a brow lift is assessed separately.
- Significant dry eye, advanced lower eyelid laxity, uncontrolled blood pressure or blood thinner use may change the plan or split it into separate sessions.
Procedures often combined with a midface lift
- Lower eyelid surgery: bag fat can be spread into the groove (fat transposition); canthopexy can support a lax lid.
- Upper eyelid surgery: often in the same session; see upper vs lower eyelid surgery.
- Brow lift, facelift, neck lift or fat grafting: when brows, jawline, neck or volume also need attention.
If only part of the face is rejuvenated, the areas may no longer match; see our natural-looking facelift guide. Session planning is decided at consultation.
Recovery after a midface lift
Recovery depends on whether the midface lift is performed alone or with other procedures, so timings are set individually. Swelling settles gradually; the final result usually takes 3–6 months to settle. Checks take place at the practice in Nişantaşı.
- With lower eyelid surgery: bruising and swelling are moderate and may spread to the cheek. Cold compresses and keeping your head raised in the first 48–72 hours help; watery, gritty or dry eyes may occur in the first days.
- With a facelift: recovery generally follows the facelift timeline. For a facelift, the clinic’s instructions place light exercise after 3–4 weeks and full exercise after 12 weeks.
For a facelift recovery timeline, see how is a facelift performed.
Risks
Through a lower eyelid incision, the risks of lower eyelid surgery apply: temporary dry or watery eyes, slight asymmetry, the lower lid being pulled down, a visible scar, bleeding or infection. Very rare bleeding behind the eye needs urgent assessment: contact your surgeon immediately about sudden pain, swelling or vision change.
Planning a midface lift from the UK or Europe
Travel, surgery and follow-up should be planned together. Before booking your trip, discuss the consultation schedule, hospital arrangements and postoperative checks with the team.
- Virtual consultation: you will be asked to send clear photos of your face from the front and profile views (and 45-degree angles). Assoc. Prof. Dr. Yalçın Bayram personally reviews your anatomy, skin quality and ageing signs; your plan is then confirmed at your in-person consultation in Istanbul before surgery.
- In Istanbul: consultations and postoperative checks take place at the practice in Nişantaşı; the operation takes place in a hospital.
- Return travel: planned after postoperative assessment by the treating surgeon. Before returning home, confirm the follow-up schedule and how to contact the clinic if you have a concern.
Factors affecting midface lift cost
The cost is determined individually after a consultation and examination. It depends on:
- The technique chosen and the extent of the procedure
- Whether eyelid surgery, a brow lift, a facelift or fat grafting is added
- The type of anaesthesia and the length of the hospital stay

Midface lift: frequently asked questions
How is a midface lift different from a facelift?
A midface lift specifically targets sagging in the cheek area, lifting the descended fat pads over the cheekbones. A facelift is a more comprehensive operation addressing the mid-face, lower face and neck. Which suits you is decided at consultation.
Is a midface lift done with lower eyelid surgery?
It is often performed in conjunction with lower eyelid surgery, through the same incision used for lower eyelid surgery. The type of incision and eyelid tension are assessed at consultation, and canthopexy may be planned if the lid is lax.
How are the technique and incision chosen?
The source and distribution of sagging, skin quality, volume loss, brow position and lower eyelid tension are assessed together. The technique and incision are decided at consultation on that basis; a midface lift is often performed with lower eyelid surgery, through the same incision.
Is fat grafting enough for sagging cheeks?
If there is no significant sagging and the concern is mainly volume loss, fat grafting may be discussed. If sagging and volume loss occur together, fat grafting can be planned with a lift or separately.
Will it soften the lines from my nose to my mouth?
Deep nasolabial folds caused by descent of the midface are among the situations in which a deep plane facelift is often considered; where volume loss is the main issue, fat grafting can be used. The right approach is assessed at consultation.
How long is recovery after a midface lift?
It depends on whether the midface lift is performed alone or combined with lower eyelid surgery or a facelift, and it is set individually at consultation. The final result usually takes 3–6 months to settle.
When can I fly home after a midface lift in Turkey?
The timing of return travel depends on the extent of surgery, your recovery and the treating surgeon’s postoperative assessment. Confirm the planned length of your stay before booking.
Where do the consultation and surgery take place?
Consultations and postoperative checks take place at the practice in Nişantaşı, Istanbul (Harbiye, Şişli). The operation takes place in a hospital; the hospital arrangements are discussed before surgery.
