Neck Lift Surgery in Istanbul, Turkey: Platysmal Bands and Jawline
Short answer: A neck lift corrects loose skin under the chin and on the neck, vertical platysmal bands and, where needed, deep fat beneath the muscle. It is performed in hospital, usually under general anaesthesia, and most patients return to social life within 1–3 weeks. Whether the neck is treated alone or with a facelift is decided at consultation.
A less defined jawline is not always caused by fat: the skin, fat, muscle and deeper structures of the neck change together. Assoc. Prof. Dr. Yalçın Bayram plans each neck lift according to which layer is causing the problem. Consultations and postoperative checks take place at his practice in Nişantaşı, Istanbul; the operation takes place in a hospital.

What Is a Neck Lift?
A neck lift is surgery in which excess neck skin is removed, the remaining skin is repositioned and, in many cases, the platysma muscle is repaired; deep fat beneath the muscle is adjusted if needed. It is not a one-size-fits-all operation: the steps depend on which layer is causing the problem.
What Can a Neck Lift Correct?
Platysmal bands
The platysma is the thin muscle covering the neck. With age its front edges loosen and separate, showing as vertical bands that stand out when you talk or clench your teeth. Correcting them may require muscle repair (platysmaplasty).
Loose skin
As collagen and elastin decrease, neck skin thins and sags. A neck lift removes excess skin and repositions the rest; liposuction removes fat only and does not tighten skin.
Deep fat beneath the muscle
Fat can sit above the muscle (subcutaneous) or beneath it (deep fat). Liposuction reaches subcutaneous fat; deep fat requires surgical access. The under-chin incision allows removal of fat beneath the platysma, reduction of the digastric muscle if needed and reduction of the submandibular (salivary) gland where necessary. Not every patient needs these steps.
Jawline and chin–neck angle
Tissue sagging, fat distribution, a descended salivary gland and bone structure all affect jawline definition. A receding chin can blur the chin–neck angle even at a young age, and a chin procedure may be discussed. A neck lift alone does not correct jowls; when they are present, a facelift is considered as well.
Double Chin Liposuction, Neck Lift or Facelift: Which When?
The SMAS layer of the cheek continues into the neck as the platysma, so jawline sagging and neck laxity usually come from the same process. Lifting only the face can leave the neck looking older; lifting only the neck can create a mismatch with the jawline. The decision is made at consultation.
| Situation | Option to consider | Incision | Limitation |
|---|---|---|---|
| Excess fat under the chin, good skin elasticity | Double chin liposuction | Millimetre-sized entry points under the chin or behind the ear | Does not tighten loose skin or treat muscle bands |
| Loose skin and bands on the neck; jawline relatively well preserved | Neck lift on its own | Under-chin incision and, if needed, incisions behind the ear | Does not correct jowls or midface sagging |
| Jawline sagging and neck laxity | Facelift and neck lift together | In front of and behind the ear into the scalp; under the chin in most patients | Longer operation and recovery |

Technique and Incision Sites
Under-chin incision and platysmaplasty
Through an incision a few centimetres long just behind the natural crease under the chin, the separated edges of the platysma are stitched together and fat beneath the muscle is contoured. This incision is made in most patients, when a platysmaplasty is needed or to sharpen the jawline.
Lateral tightening
The platysma can also be repositioned towards the area behind the ear, with excess skin removed through incisions around the ear. In mild laxity, especially in young patients with little excess skin, tightening from the sides alone, without a platysmaplasty, may be enough.
The role of liposuction
Liposuction is often part of a neck lift. Because removing too much fat can leave a hollowed or uneven neck, fat contouring is planned in moderation.
Every incision leaves a scar; in most patients it becomes less noticeable over time. For scar placement and care, see will a facelift leave a scar?; for the stages of surgery, see how is a facelift performed?
Is a Neck Lift Right for You?
A neck lift suits people whose sagging comes not only from fat but also from loosening skin and muscle. Candidates usually have several of the following:
- Loose skin under the chin and on the neck
- Visible vertical platysmal bands
- Fullness under the chin that remains despite diet and exercise
- A stable weight and general health that allows surgery
- Not smoking, or being able to stop around the time of surgery
- Realistic expectations and time for recovery
Who may not be suited?
- People with uncontrolled diabetes, cardiovascular disease or a bleeding disorder
- People who cannot pause blood thinners with their doctor’s approval, or cannot stop smoking
- People still losing weight or planning a major weight change
- People with good skin elasticity and fat only under the chin; a more limited procedure such as double chin liposuction may be enough
Age alone is not the deciding factor. More detail: who is a neck lift suitable for?
Neck Lift Recovery
- First few days: Swelling, bruising and tightness are expected. Keep your head raised, avoid bending your neck excessively and wear the recommended neck bandage. A thin drain, if used, is usually removed within 1–2 days.
- Weeks 1–2: Swelling and bruising reduce noticeably; removable stitches, if used, come out around day 10. Most patients return to desk work by the end of this period.
- Weeks 3–4: Light exercise may begin, in line with the surgeon’s advice.
- After 12 weeks: Return to full exercise.
- Months 3–6: Remaining swelling settles, the jaw–neck line becomes clearer and scars continue to mature.
Temporary numbness or firmness in the neck may occur and usually improves over time.
Risks
Serious complications are uncommon, but risks include haematoma, seroma and infection, usually temporary weakness of the nerve branch that moves the lower lip, contour irregularity, asymmetry, scar thickening and skin circulation problems, particularly in smokers. Results vary from person to person.
Consultation at the Practice, Surgery in Hospital
Consultations and postoperative checks take place at the practice in Nişantaşı (Harbiye, Şişli, Istanbul); the operation takes place in a hospital, and the hospital arrangements are discussed before surgery. At consultation, skin elasticity, where the fat lies, how the bands behave when you clench your teeth, jawline sagging, chin structure and the salivary gland are assessed, together with your health, medications and smoking.
Planning Your Neck Lift Trip to Istanbul
Travel, surgery and follow-up should be planned together. Before booking your trip, discuss the consultation schedule, hospital arrangements and postoperative checks with the team.
- Virtual consultation: Before booking your flight, you will be asked to send clear photos of your face from the front and profile views (and 45-degree angles). Assoc. Prof. Dr. Yalçın Bayram personally reviews your anatomy, skin quality and ageing signs; your plan is then confirmed at your in-person consultation in Istanbul before surgery.
- Length of stay: Confirm your consultation dates, accommodation and transport before travelling. Plan your stay around the hospital nights and early check-ups, including stitch removal if needed, rather than the operation date alone.
- Flying home: Return travel is planned after postoperative assessment by the treating surgeon. Before returning home, confirm the follow-up schedule and how to contact the clinic if you have a concern.
What Affects the Cost of a Neck Lift in Turkey?
The cost is determined individually after a consultation and examination. The main factors are:
- The extent of surgery: the neck alone or combined with a facelift
- Whether platysmaplasty, deep fat contouring or liposuction is needed
- Whether other procedures, such as eyelid surgery, are combined
- The type of anaesthesia and the length of the hospital stay
- The standards of the hospital

Frequently Asked Questions
How long does neck lift surgery take, and will I stay in hospital?
It depends on whether the neck is treated alone or with a facelift. When the face and neck are treated together, surgery mostly takes a few hours and two nights in hospital are planned in most cases. For a neck lift alone, the duration and hospital stay are set at consultation.
Will I need a neck bandage or a drain?
Wearing the neck bandage your surgeon recommends is advised in the early period; how long you wear it depends on the extent of surgery. A thin drain may be placed in some patients and, if used, is usually removed within 1–2 days.
When are the stitches removed after a neck lift?
Dissolvable stitches are mostly used. Removable stitches, if used, usually come out around day 10, so plan your stay in Istanbul with this in mind.
When can I exercise again after a neck lift?
Light exercise may begin after 3–4 weeks, in line with the surgeon’s advice, and full exercise after 12 weeks. Most patients return to desk work within 1–2 weeks.
Is an incision under the chin always needed?
Not always. An under-chin incision is made in most patients, when the platysma needs repair or to sharpen the jawline. In mild laxity, especially in young patients with little excess skin, tightening from the sides alone may be enough; this is decided at consultation.
Is numbness normal after a neck lift?
Temporary numbness or firmness in the neck may occur and usually improves over time. Remaining swelling settles and the jaw–neck line becomes clearer within 3–6 months.
When can I fly home after a neck lift in Turkey?
The timing of return travel depends on the extent of surgery, your recovery and the treating surgeon’s postoperative assessment. Confirm the planned length of your stay before booking, and follow the surgeon’s advice before flying.
